Could hormones influence how women respond to psilocybin and other psychedelics? New research is drawing attention to an important but surprisingly underexplored area of psychedelic science: the role of female sex hormones.

Interest in psilocybin research has grown enormously in recent years, with scientists investigating everything from its effects on the brain to its potential role in future approaches to mental health.

But there is another variable researchers are beginning to look at more closely: hormones.

A recent scientific paper published in Molecular Psychiatry explores the potential relationship between female sex hormones and psychedelic effects, raising an intriguing question:

At the moment, there isn’t enough evidence to give us a definitive answer. But researchers argue there are good scientific reasons to investigate.

Why Are Researchers Interested in Hormones and Psychedelics?

Two particularly important hormones are estradiol, a form of oestrogen, and progesterone.

Levels of these hormones can change considerably during:

What About Hormonal Contraception?

Hormonal contraception adds another interesting layer.

Millions of women use contraceptive methods that alter normal hormonal patterns, yet this isn’t necessarily treated as a major variable in psychedelic studies.

Researchers are questioning whether future trials should examine participants using hormonal contraception separately from those experiencing naturally fluctuating menstrual cycles.

Again, there isn’t currently enough evidence to say that contraception changes a person’s response to psilocybin.

The point is that researchers may need to start measuring these differences before we can know whether they matter.

Pregnancy and the Postpartum Period

Pregnancy and the months following birth involve some of the most dramatic hormonal changes experienced during adult life.

They are also periods in which vulnerability to certain mental health conditions can change.

As psychedelic-assisted therapies continue to be investigated, researchers argue that women’s reproductive health needs to become a more prominent part of the conversation.

This doesn’t mean psychedelics have been demonstrated to be safe or effective during pregnancy or breastfeeding – they have not.

Instead, it highlights how much remains unknown and why carefully designed research into women’s mental health is important.

What About Perimenopause and Menopause?

Another potentially significant area is menopause.

During perimenopause, levels of hormones including oestrogen and progesterone can fluctuate considerably before eventually declining.

These hormonal changes can coincide with changes in mood, sleep, cognition and emotional wellbeing.

At the same time, psychedelic research is increasingly investigating conditions that can affect people during midlife.

Yet the interaction between menopause, hormonal status and psychedelic response remains poorly understood.

Future research may therefore need to consider menopausal status and hormone therapy when analysing psychedelic clinical trials.

Could Hormones Affect How Psilocybin Is Processed by the Body?

The relationship might not be limited to the brain.

Hormones can influence several processes involved in how drugs move through and are processed by the body, including gastrointestinal function and liver enzyme activity.

Researchers have therefore proposed that hormonal variation could potentially affect the pharmacokinetics of psychedelics – essentially how a compound is absorbed, distributed, metabolised and eliminated.

For psilocybin specifically, however, direct human evidence demonstrating a meaningful hormonal effect on its pharmacokinetics is currently lacking.

This is another area where the question is currently more developed than the answer.

Why Women’s Psychedelic Research Matters

The wider issue highlighted by this research extends beyond psilocybin.

Historically, biological differences associated with female reproductive hormones haven’t always been adequately incorporated into medical research.

But treating women as one biologically static group can overlook substantial changes occurring across the lifespan.

A 25-year-old using hormonal contraception, a woman in the postpartum period and someone experiencing perimenopause may have very different hormonal environments.

We don’t know yet.

But the direct human evidence remains limited.

Rather than providing a definitive answer, this research identifies a gap in our understanding – and provides scientists with a roadmap for investigating it.

Future psychedelic studies may increasingly need to consider factors including menstrual cycle stage, hormonal contraception, pregnancy and postpartum status, menopause and hormone therapies.

Doing so could help researchers build a much more detailed understanding of why psychedelic experiences and clinical outcomes may vary between individuals.

The Next Chapter of Psychedelic Research?

Psychedelic science has advanced rapidly, but increasingly sophisticated research is revealing just how many questions remain unanswered.

The next stage may not simply be about asking whether psilocybin has an effect.

It may be about understanding why effects differ between people – and which biological factors influence those differences.

Hormones could prove to be one piece of that puzzle.

For now, the evidence isn’t strong enough to draw firm conclusions.

But the fact researchers are beginning to ask these questions represents an important step towards a more nuanced understanding of psychedelics, the brain and women’s mental health.
