Psilocybin and ADHD: Encouraging Signs From Early Research

Interest in psilocybin and its possible effects on mental health has grown considerably in recent years. Alongside research into depression, anxiety and addiction, scientists are beginning to examine whether psychedelic substances could also influence symptoms associated with attention deficit hyperactivity disorder, or ADHD.

Some of the earliest findings are encouraging. Adults experimenting with psychedelic microdosing have reported improvements in ADHD symptoms, mindfulness, emotional regulation and overall wellbeing.

However, this is still a very new field of study. Psilocybin has not been proven to treat ADHD, and most of the positive evidence currently comes from observational research rather than controlled clinical trials.

So, what promising signs have researchers identified so far?

Understanding ADHD

ADHD is a neurodevelopmental condition commonly associated with difficulties involving attention, impulsivity and hyperactivity.

Its impact can extend far beyond concentration. Adults with ADHD may also experience difficulties with:

  • Organisation and time management
  • Motivation and task completion
  • Emotional regulation
  • Stress and frustration
  • Relationships
  • Self-esteem and general wellbeing

Established treatments include stimulant and non-stimulant medication, psychological therapies and practical support. These approaches can be extremely effective, but they do not work equally well for everyone.

This has encouraged researchers to investigate new possibilities, including psychedelic-assisted therapy and microdosing.

What is psilocybin microdosing?

Psilocybin is a naturally occurring psychedelic compound found in certain species of mushroom.

Microdosing generally involves taking a small amount of a psychedelic substance that is intended to remain below the level required to produce a full psychedelic experience. Those who microdose often report doing so in the hope of improving their mood, concentration, creativity or emotional wellbeing.

Research involving adults with ADHD has largely focused on self-directed microdosing rather than clinically supervised, full-dose psilocybin therapy.

Reduced self-reported ADHD symptoms

One of the most encouraging findings comes from a prospective study that followed adults with diagnosed ADHD or severe ADHD symptoms.

Participants had already decided to begin microdosing independently. Researchers assessed them before starting and again after two and four weeks.

The participants reported a reduction in ADHD symptoms after beginning microdosing. Interestingly, those using conventional ADHD medication alongside microdosing did not report significantly different outcomes from those who were not using medication.

This suggests that the changes were not simply restricted to people taking established medication at the same time.

The substances used were not fully standardised, although approximately 78% of participants reported microdosing psilocybin-containing mushrooms or truffles.

While the results cannot prove that psilocybin caused the improvements, they provide an early signal that deserves further investigation.

Potential improvements in mindfulness

A related study examined mindfulness and personality characteristics in the same general population.

After four weeks of self-directed psychedelic microdosing, participants reported increased mindfulness. The most noticeable changes involved their ability to describe their internal experiences and respond to thoughts and feelings without immediately judging them.

This could be particularly relevant to ADHD. Mindfulness involves recognising where attention has moved and gently returning it to the present moment. It may also help people notice strong emotions without reacting to them automatically.

The researchers found that mindfulness had improved after two weeks and increased further by the four-week assessment.

Although microdosing cannot yet be credited as the cause, the pattern suggests a potentially valuable area for controlled research.

Lower levels of neuroticism

The same study also recorded a reduction in neuroticism after four weeks.

Neuroticism is a personality trait associated with a greater tendency to experience anxiety, worry, emotional instability and vulnerability to stress. On average, adults with ADHD tend to score more highly for this trait than people without the condition.

A reduction could indicate improved emotional resilience or a less reactive relationship with difficult thoughts and feelings.

Other personality characteristics did not change significantly, suggesting that the result was not simply a general improvement across every measurement.

Personality traits are generally considered relatively stable, so observing a change in neuroticism was especially interesting. However, researchers stressed that placebo-controlled trials are needed to establish whether the substance itself produced the effect.

Encouraging signs for emotional regulation

Emotional dysregulation is not always included in simple descriptions of ADHD, but it can have a significant impact on daily life.

People may experience emotions very intensely, become overwhelmed quickly or struggle to regulate frustration, rejection and stress. Conventional ADHD medication can help some people, although its effects on emotional regulation may be less consistent than its effects on attention and hyperactivity.

A 2024 prospective study examined psychedelic microdosing, emotional regulation and empathy in adults experiencing severe ADHD symptoms.

During the first part of the research, participants reported improvements in two emotional-regulation strategies:

  • Cognitive reappraisal, which involves changing how a situation is interpreted
  • Expressive suppression, which involves managing how emotions are outwardly expressed

They also reported changes in perspective-taking and personal distress.

When researchers introduced a comparison group, most of the apparent emotional and empathy-related benefits were no longer statistically significant. Improvement in expressive suppression did remain, however.

The findings are therefore mixed, but they suggest that emotional regulation may be one of the most valuable areas for future ADHD and psilocybin research.

Lower symptom scores than the medication group

The second part of the 2024 study compared adults who were microdosing with a much smaller group using conventional ADHD medication.

After four weeks, the microdosing group reported lower ADHD symptom scores than the medication group.

This is an eye-catching finding, but it does not demonstrate that microdosing works better than ADHD medication.

Participants were not randomly assigned to the two approaches, the groups were very different in size and the substances and doses used for microdosing were not clinically controlled. The people who chose to microdose may also have differed from those using medication in ways that influenced the results.

Nevertheless, the reduction in reported symptoms gives researchers a reason to investigate the relationship through properly controlled trials.

Improvements beyond concentration

One of the most interesting aspects of this research is the range of changes reported.

The emerging signals do not relate only to concentration. They include:

  • Lower self-reported ADHD symptoms
  • Increased mindfulness
  • Reduced neuroticism
  • Possible improvements in emotional regulation
  • Improved general wellbeing

This matters because ADHD can affect far more than the ability to focus. If psilocybin research continues, scientists may need to examine its effect on the complete experience of ADHD, including emotional wellbeing, self-awareness and everyday functioning.

It will also be important to determine whether any improvements in attention happen directly or result from reduced stress, anxiety or emotional overload.

Why might psilocybin have an effect?

Researchers do not yet know whether psilocybin can improve ADHD symptoms or precisely how any benefit might occur.

Psilocybin primarily affects the serotonin system, particularly the 5-HT2A receptor. This differs from conventional stimulant medications, which mainly influence dopamine and noradrenaline.

Psychedelic research in other mental health conditions has associated psilocybin with temporary changes in communication between brain networks and possible increases in psychological flexibility.

In theory, greater flexibility could help people move away from rigid patterns of thought or behaviour. Increased mindfulness might also support emotional awareness and intentional responses.

These are plausible areas for research, not confirmed explanations for the improvements reported by people with ADHD.

What are the limitations?

Despite the positive signs, the existing evidence has significant limitations.

Most ADHD-specific studies have involved people who decided for themselves that they wanted to microdose. Participants knew they were taking a psychedelic, making expectation and placebo effects difficult to separate from the substance’s pharmacological effects.

Other limitations include:

  • Small numbers completing the final assessments
  • High dropout rates
  • Reliance on self-reported symptoms
  • No standardised dose or substance
  • Limited objective testing of attention
  • Short study periods
  • A lack of credible placebo groups
  • Participants selecting their own treatment approach

The improvements reported by participants may be genuine and meaningful. The current research simply cannot confirm how much of the change was directly caused by psilocybin.

A 2026 systematic review concluded that uncontrolled studies have produced potentially positive short-term signals, but that the overall evidence remains insufficient to establish psychedelics as an ADHD treatment.

What research is needed next?

The early findings provide several promising leads, but they now need to be tested under controlled conditions.

Future studies should include:

  • Larger and more diverse groups
  • Laboratory-tested psilocybin
  • Standardised doses
  • Random assignment to psilocybin or placebo
  • Clinician assessments as well as self-reports
  • Objective measurements of attention and executive function
  • Longer follow-up periods
  • Careful monitoring of adverse effects
  • Research into possible interactions with ADHD medication

Researchers should also investigate microdosing separately from full-dose, professionally supported psychedelic therapy. These are very different approaches and may produce different effects.

Promising, but not yet proven

The early research contains some genuinely encouraging signs. Adults who began psychedelic microdosing reported reductions in ADHD symptoms alongside improvements in mindfulness, emotional regulation and aspects of psychological wellbeing.

These findings give scientists good reasons to continue investigating psilocybin and ADHD. They do not yet provide evidence that psilocybin is a safe or effective ADHD treatment.

Psilocybin should not be used as a substitute for professional assessment, prescribed medication or established support. Anyone considering changing their ADHD treatment should speak to a qualified healthcare professional.
